How It Works

♦Bring your pet in for a Physical Exam.

♦Enroll your pet in the Free Vaccine For Life Program by paying a one-time affordable enrollment fee.

♦You agree to bring your pet back to Cheek to Cheek Hospital for Animals at least once every 12 months for a Physical Exam.

♦Your pet receives the core vaccines recommended by the American Animal Hospital Association FREE for the rest of his or her life.

What Else Should I Know?

♦  All examinations, diagnostic tests, services and products other than vaccines will be charged for according to our regular fee schedule.

♦Enrollment is not transferable and not exchangeable between pets or owners.  The enrollment fee is not discountable or refundable.

♦There is a 30 day grace period for your pet's annual exam.  If you fail to have the yearly exam performed within this period, enrollment or membership expires.

♦Other vaccines such as Canine Influenza, Lyme Disease and Leptospirosis are not included.
